Download the Forky headband template, print and cut out the pieces.

Assemble Forky's eyes by gluing the pupils slightly off-centered on each eyeball.

Position the eyes in the middle of the headband and glue in place.

Outline the eyes with the black marker.

Glue Forky's red eyebrows just below the spork prongs.

Add as many extenders as you need to fit your child.
